UnityPoint Health — Trinity

according to state guidelines. You can schedule your vaccine appointment without having to hear from them first:

  • New appointments are being accepted now for Express – Duck Creek in Bettendorf for anyone age 16 and older, and Express Care – North Port in Muscatine for anyone age 18 and older. Residents of Illinois or Iowa may register for either location. The Duck Creek clinic is at 1417 Kimberly Road.
  • A new online scheduling tool at www.unitypoint.org/quadcities/covid-19-vaccine.aspx is a quick and easy way to get your vaccine appointment scheduled. Just select the location, date and time that works best for you.
  • You can call 309-779-7979 to schedule your appointment for either clinic location.
  • Current patients of UnityPoint Clinic may also sign up through MyUnityPoint. Click on “Schedule an Appointment” and select the “Covid-19 Vaccine” option to select a vaccine appointment.
  • The second dose should be scheduled at least 21 days (Pfizer) or 28 days (Moderna) from your first appointment. If you attempt to schedule an appointment prior to the 21 or 28 day window, you will receive an error message. Please re-click the initial link and adjust the date accordingly. You can schedule your second dose at your first appointment, or you will receive a text following your first appointment to schedule.
  • The Moderna vaccine is only authorized for individuals 18 and older. The Pfizer vaccine is authorized for individuals 16 and older. If Pfizer is available and an individual 16 or 17 years of age would like to schedule, parent or guardian consent is required. A parent or guardian must also accompany patients under 18 at the Covid appointment.

UnityPoint is not accepting walk-in Covid-19 vaccinations at this time, but is encouraging the community to sign up to receive the vaccine as soon as possible.

“Receiving the Covid-19 vaccine is the best way to help our community reduce Covid-19 infections,” according to a UnityPoint release.
